South Africa Builds Africa’s Healthcare Supply Chains as USD 2 billion Flows Into the Sector
South African manufacturers and distributors are capturing a growing share of continental healthcare demand, with African Continental Free Trade Area (AfCFTA) reshaping how countries buy essential healthcare products and the African Pooled Procurement Mechanism (APPM) moving from policy to implementation.
In February 2026, African Union (AU) leaders committed to fully operationalising APPM as a strategic “Buy African” initiative through demand aggregation, market shaping and long-term offtake arrangements, By May, the Africa CDC had completed its first APPM tender across 10 AU Member States, securing 30–90% lower prices and involving African manufacturers in five of the 10 products tendered. In June, Egypt became the first AU Member State to formally join APPM and agreed to host its operational headquarters, underlining the mechanism’s shift to continent-wide deployment.
More than R 33 billion (USD 2 billion) in new healthcare investment is flowing into African markets through development finance institutions, private equity and blended-finance vehicles. As South Africa’s operational and financial centre, Johannesburg is where much of that capital is deployed, financed and managed.
Taking place from 6 – 8 October 2026 at the Gallagher Convention Centre, the World Health Expo (WHX) in Johannesburg will bring the continent’s healthcare procurement and investment leaders into one room: the most direct route to market for manufacturers and investors serious about African trade. With 13 country pavilions and a dedicated Laboratory Zone, more than 600 exhibitors from 60+ countries, and over 10,000 professional visits across a 15,000 net sqm show floor, WHX is positioned at the intersection of South Africa’s healthcare investment corridor and uniquely timed to connect local manufacturers and continental procurement decision-makers and investors.
South Africa’s stronghold in the continent’s medical device market reflects more than import volume. It sits at the centre of a two-door market structure (public tenders and private hospital groups) and suppliers that succeed are the ones able to work both channels in parallel. Johannesburg is the functional heart of South Africa’s healthcare economy home to the major business headquarters and distributors that drive the sector forward.
As well as being a major buyer, South Africa offers a gateway for the continent’s health trade. The market’s sophistication comes from a mature regulation, established procurement systems, and a dense ecosystem of logistics, engineering, warehousing and service providers that support distribution into Southern African markets and beyond.
The AfCFTA and the AfCFTA-anchored Pharmaceutical Initiative has created a framework for regional pooled procurement of priority health products, leveraging rules of origin, harmonised regulation and integrated trade to anchor health spending within African markets. In 2024, AU Heads of State endorsed full operationalisation of the APPM, mandating Africa CDC to run a continent-wide pooled procurement system that prioritises African-based suppliers.

This will be complemented by three dedicated Healthcare Indabas: the Healthcare Excellence Indaba on 6 October (chaired by Prof Edward Kunonga, Strategic Advisor, Global Health), the Labs Leadership Indaba on 7 October, in partnership with NHLS (chaired by Prof Koleka Mlisana, Chief Executive Officer, National Health Laboratory Service) and the Quality Management & Patient Safety Indaba on 8 October, in partnership with COHSASA (chaired by Professor Sabelile Tenza, Associate Professor, School of Nursing Science).
In addition, the exhibition will showcase a variety of business workshops as well as nine product categories spanning medical devices and equipment, disposables and consumer goods, healthcare and general services, imaging and diagnostics, orthopaedics and physiotherapy, IT solutions, healthcare infrastructure, wellness and prevention, and laboratory technologies.
